221(g) white slip asking for legal documents on Name and DOB change

I got my passport back with 221(g) white slip asking for “Legal documents explaining Name and DOB change and asking me to appear in person. But I never changed my name or DOB in the past or recently. I checked my DS160 as well and it all has the right information. Please help here.

Is there a name mismatch anywhere in any document like passport and visa application?

What type of visa have you applied?

I have cross checked everything there was no mismatch. I have dropped my passport for H1B stamping.
