Adding surname on Indian passport

Hi all,

My wife’s current passport only has a “Given Name” and no surname. We are trying to apply for a re-issue of passport and adding my surname to the passport. I believe it would be considered a change in name due to marriage (and I can provide marriage certificate as a proof).

I was wondering if I need to fill in the “Name Change Affidavit”?

Name change on account of marriage does not require affidavit. Provide a self attested copy of marriage certificate.
