Hello. I’m renewing my Indian passport from USA. As a part of checklist, I need to fill change of appearance, Annexure D.

I want to print my Indian address on the passport. So, which address do I need to mention in the change of appearance and Annexure D forms?

That should be your USA address since you physically present here in USA.
