COS from H1B to H4 and then back to H1B

As clearly mentioned in the H1B 60 day grace period article, you get either 60 days or i94 expiry days, whichever is lower.

Each new H1B extension comes with a new i94.

There are no documents or application required to file grace period - this has also been clearly mentioned on the article.
