My wife who was H4 dependent on me recently got her H1B approved from her employer in Oct 2024. She is now in H1B status. She doesn’t have a H1B visa stamp on her passport.
She is now in India. She has her previous H4 visa stamp valid till Oct 2025. She wants to return back to US using her old H4 visa stamp and continue to be on H4. Please let me know if she can do that.

Yes, she can use old H4 visa stamp to enter USA.
Thank you so much for your response @anil_am22. Is there anything she needs to be prepared to mention to CBP officer at port of entry?
Just want to make sure they wont question why she is coming in with H4 while her current status is H1
