Have never refused visa ( unless such refusal was overcome or waived )
I got 221G for my dropbox last year and went for interview and it was approved. What should be my answer for this question ?
For dropbox, this question can be answered ‘no’ if visa was finally approved.

btw, I did check the app but it seems it does not have 12month rule check, does it?
It has 12 month rule. Did you enter correct date of dropping the document?
You are right, I did not notice the end date in the app ( was trying in the phone, now tried on desktop) It was taking 3 years by default, which made me thing it did not account for 12 month rule.
All good, but I think app should highlight end date as a mandatory field, right ? in some cases it could be 1 year etc. Just a thought.
