H4 EAD independent contractor DS160

Hi - my wife has h4 ead and worked in the past as an Independent Contractor for xyz company and they sent her 1099-nec for the worked years. She is travelling to India this year and had question related to employer details in DS160.

In this scenario under ds160 past employment section- should she enter employer name as “self employed” and home address as employer address or she should put xyz company details who release her 1099?

1099 is considered self employment. You should write your own work address and ‘self employed’.
