H4 refused in dropbox

I came to india for h1b for me and h4 visa stamping for my wife. Now h4 dropbox status is showing as refused while h1b is showing as approved.

What should be the next step? Will h4 be approved? How many days should i wait since I already have my return booked? Is refused an intermediate step before approval especially since h1 is approved?

Status is showing as approved since 6 May. Do you know how long it takes to change to issued ?
