Help with indian address Self Attested and other address

hey i have not been to India in past couple of years (which is more than 5 years )and my passport is about to expire while filling out the form for renewal I came across a couple of qns

1. in self-sworn-aff-Indian-address should the Indian address be the last known address of mine I,e the address in my current passport (which matches all my ids in India ) or it should be the latest address of my parents or siblings?
   

   

2. I gave the primer address as a USA address should the other address be the same as the address mentioned in the self-sworn-aff-Indian-address or it can be the new address of my relatives

Indian address should be the most recent and active address. This can be your relative address as well.

USA address should also be the current active address.
