Indian Passport Renewal - Consent Letter format for no change in Parent name

Hi,
I applied for Indian Passport renewal in USA. I got ONHOLD status with “PARENT`S NAME DOES NOT MATCH WITH PASSPORT”.
Solution provided is to create SIGNED CONSEMT LETTER to restore same as in Passport. I have couple of questions.

  1. Do we have any template for consent letter? Please share.
  2. Option is provide to upload the consent letter along with email receive online with Payment , can I do that?
