Indian passport renewal for minor/child - How to make child sign?

Hello, how does one sign Annexure E and the Affidavit for Change in appearance and signature, for a 4 yr old child. Should I have him place his thumb impression, or should one of the parents sign instead? Reason I ask is Annex E and the Affidavits are declarations by the applicant, not parents.

Also, I indicated a change of address because my son’s first passport has our Indian address (he was born in India). However, since we are residing in the US, I wanted his new passport to show our US address. Now, do I need to provide address proof for US residence only (which is easy), or do I need to provide docs for Indian address proof as well? Because the list of documents contains one for Indian address proof.

Child Indian Passport Renewal - Self Attestation and Notarize

SELF-ATTEST – just sign yourself along with your child.
If Child > 5 years, should write his/her name them-self.
If your child is less than 5 years, he/she should use his/her thumb print.
Both parent and child need to self-attest.
Each parent should self-attest their OWN document . Get your child to sign all documents too.
Mother should sign her OWN and Father his own. Mother CANNOT sign father’s document and vice versa.
For general document (like forms), any one parent can sign.

  1. CHILD’s – Passport copies of First five and last two pages
  2. CHILD’s – Birth Certificate
  3. US Address proof – Utility bill or driving license or rental lease agreement on any one of parent’s name.
  4. Indian Address proof – Utility bill or driving license or rental lease agreement on any one of parent’s name.
  5. Mother’s – Color copy of passport pages that you have photocopied.
  6. Father’s – Color copy of passport pages that you have photocopied.
  7. Mother’s – Color copy of Visa or Green Card or I797 approval notice.
  8. Father’s – Color copy of Visa or Green Card or I797 approval notice.
  9. ANNEXURE ‘D’ – Affidavit for Applying Minor passport by parents
  10. Annexure ‘E’ – Specimen Declaration of the applicant on a plain paper
  11. CHILD’s – Color copy of Visa or Green Card or I797 approval notice.

Indian Address Proof

Indian address proof is not required if you are opting to get US address printed on new passport.

In the checklist I got from CKGS site, besides self-attestation, the below documents must be notarized too:

  1. Mother’s & father’s driver’s license (when used as US Address proof)
  2. Mother’ & father’s US visa
  3. Annexure D
  4. Change of Appearance / Signature Form
  5. Passport Application Form (Notary seal on pg 1 & pg 4)

That makes it a total of 8 notary seals = $6x8 = $48 (At UPS)

It does not matter if child “signature” is inconsistent when signing in multiple places. Seems ridiculous… but it is what it is.

For minor passport renewal on “AFFIDAVIT FOR CHANGE IN APPEARANCE AND SIGNATURE” there is a section of Signature “Dependent/Applicant signature”. Who needs to sign on this section, shall my son who is less than 5 years put left thumb impression or only one parent can sign or do I need to do both.

Please suggest.

Your son can write his name and both parent can sign.
