This is Ashish and have a question for Birth Certificate.In my birth certificate and educational documents my last name is missing.
In 2005 I when I applied for passport I added my last name. For this I advertised in 2 newspapers as well. My passport has my first and last name correctly mentioned.
I tried to get my last name added to my birth certificate in my state. Its a long process where I need to get all educational documents updated before that.
Could you please let me know what are my options here in USA. Is that something which can be solved with an Affidavit. If yes could you please let me know any format for that.
