Mother's name on DS 160

@anil_am22 Hi, could you please assist me with this inquiry. I’m traveling with my parents to India for visa stamping.
On my passport, on the last page ‘Mother’s Name’ is with a middle name (AAA BBB CCC), but on my mother’s passport ‘Given Name’ is without a middle name (Given Name - AAA Surname - CCC)

  1. On my DS 160 form under the ‘Family section’, should my mother’s name be listed as it appears on my passport or as it appears on my mother’s passport?
  2. Under Travel Companion Information - ‘Persons traveling with me’ section, should my mother’s name be listed as per my mother’s passport ‘Given name’ as she too is traveling with me or my passport last page?
    Do I have to list the name uniformly for consistency in my form? If so, please advise which one to use. Thank you.

I suggest to use your mother’s name as per her passport everywhere.
