Question about first five pages and last two pages of passport

I didn’t find any post about this, so just want to make sure I’m getting it right.

  1. Are they asking page number 1-5, or is it from page 0 (which is passport bio)?
  2. Are these copies need to be in separate pages? Because I have to cover the other page to scan one page at a time instead of full face including two sides.

And in case if anyone knows, why do they need first five pages? My passport doesn’t really have anything at the beginning.
