Questions for Passport Reissue Filing - Slightly Damaged

Hi

I have a couple of questions - hoping someone can respond

a) I am filing for a passport reissue (barcode is slightly smudged) - one of the checklist items says name change affidavit. If I am not changing my name, is that still needed

b) I am also adding my spouse’s name for the first time - her passport doesn’t have my name in it yet. So, can I just give the marriage certificate as a supporting document. Is there any other form / affidavit I have to file in. The form didn’t give me any options to select change in existing particulars section that I saw on some other posts.

VFS generates the document list based on the options you select for your passport. You must have selected something that made it 'name change affidavit ’ as a required document.

Marriage certificate is enough for adding spouse name in addition to spouse passport copy.
