Renew Indian passport for minors

My kid has got her passport when she was 1 month old. She is 5 now. Her passport is due for renewal.

Question 1: what is self sworn affidavit? Is it needed for minors?

I also got the checklist.
My kid can’t sign so using thumb impression.

Question 2: For minors is it mandatory to get “affidavit for change of appearance and signature” form? If yes, where can I get sample of that form?
