Hi,

Am in need of sample name difference affidavit format for acs skill accessment as my name in degree certificates are YUVA RANI R but in passport and birth certificate it’s being YUVARANI RAVISEKAR and YUVARANI R. Appreciate the assistance to get through this.

Could you please check does the attached affidavit looks okay. Personally i felt its too much of information, do i need to mention the discrepency in Payslip and Form-16 as well.Just during the document collection process realised my name being different in most of the places. Please guide me.

Am in process of filling form 80. Apart from degree certificates,payslips and form 16 all other government related documents and identity documents like passport, Aadhar card, birth certificate my name is same as YUVARANI.

Now, while filling form 80 questions 1 and 4 what should I mention? Should I mention all name diffferences or just as YUVARANI.

Please assist.

Write your Name as you have in your Current Passport.

Form 80 Question 4

Write your other names versions or alias that you have.
