Hi Friends,
I am renewing my Passport and in my OLD Passport my name as below and want to correct as New name in Passport. So as per the steps, need to fill the Affidavit for Change in Name by selecting Disclaimer-5, (ii) splitting name.
But when filling this affidavit, it looks very confusing as new name and old name don’t have section for Given name , and Surname.
If anyone has recently corrected/splitted the name, please share the sample of Affidavit which worked.
OLD:
Given Name : AAA ZZZ
Sur Name:
NEW Name :
Given Name : AAA
Sur Name: ZZZ
