Trouble finding Annexure E, Affidavit for change of appearance, self sworn affidavit

Hi,
Can you please help me on where can I download the Annexure E, Affidavit for change of appearance, self sworn affidavit. I was not able to find it on Govt. of India site as well as vfs global site. Also, which of these documents need to be notarized.

When u select passport renewal option - fresh/reissue you should see overview - click on it and click documents checklist - the links are within checklist

I am not able to find it still. Is it on VFS Global site (https://row1.vfsglobal.com/IHCUSPostalAppointment/Applicant/ApplicantList) or Passport Seva portal (https://portal4.passportindia.gov.in/Online/secure/loginAction)

There are links provided in the checklist on the application confirmation form, but they are not hyperlinks (not working).

Can you please confirm if I have understood correctly on which documents will need to be notarized and sent:

  1. Annexure E - original signed
  2. AFFIDAVIT FOR CHANGE IN APPEARANCE AND SIGNATURE - Original and Notarized (To be sent even if appearance and signature has not changed)
  3. Self Sworn in Affidavit - original signed
  4. Valid I-797 Approval Notice - Color Copy - Notarized
  5. Drivers Licence - Color Copy - Notarized
  6. 2 Photographs (1 on Govt. Application Form and 1 on Affidavit for Change in Appearance)
  7. Copies of the first five and last two pages of your current passport along with original passport.
  8. Application Confirmation Letter
  9. Aadhar Card - Indian Address Proof - Self Attested
  10. Signed Checklist

Also, should I be sending my spouses passport copy and driving licence.

Notarized Annexure E - they are not clear about it - but I have read some VFS centers ask for notarized copy - I sent it notarized. Also send your return label also. I attached it to application confirmation letter and indicated on a sticky note for their attention.

Make sure application form is printed in portrait mode.

I need suggestion for filling affidavit and self sworn for Indian passport renewal.
VFS allow to download forms, however I was unable to see option to edit form online.
Please suggest, if forms need to be filled manually or is there way to edit above mentioned 2 forms online.
Also, Annexure E allows to edit, however it does not print whole details.
Any suggestion will be helpful.

Are the Annexure E, Self Sworn Affidavit and Change in appearance forms to be filled up by hand? Do all of them need to be notarized?

I didn’t fill them by hand. I typed them. Just signature, date, place filled by hand. All 3 to be notarized. And this worked for me.

Hi Bhakti, Did you write the details in the self sworn affidavit and other documents that need to be notarized. I have prepared all my documents and I saw this note in vfs global application where is it is mentioned ‘HAND Written Documents won’t be accepted’ what does this really mean.

Moreover I tried editing the documents and the prints do not come in with entire details as expected especially with the ‘Annexure E’

I filled in all forms by hand. I did not face any issues with VFS.
