[URGENT REQUEST] Consent Letter Requested for Passport Reissue

Hello, I received an email from VFS saying my application was on hold, with this request –

Additional documents : Please provide the consent letter to keep the father’s
name same as current passport ( Under tatkal application No changes allowed)

Are they looking for my consent to use my father’s name as is? Or do they need his signature? What should this letter say? Do i have to get this letter notarized? Can i just mail this additional consent letter, or do i have to include the application page again?

I’m going to be traveling to India in 2 weeks, so any immediate help will be much appreciated. Thank you!!

They just need the consent letter to print the father name on new passport as is.

Right, can the letter just say:
“I consent to print my father’s name on the new passport as it is on the current one?”
