YOu mentioned that Annexure E and Self SWORN Affidavit need to be Notarized, but that’s not the case on the passport checklist provided by VFS…Can you confirm?

I have been trying to fill a passport renewal application in the portal4.passportindia.gov.in website.
The website is extremely bugged. It has been giving me issues for the past few days. I tried different browsers, computers and checked internet speed etc. Passport is expiring in 4 weeks, and I am concerned the website will not be fixed due to the Covid crisis. Thanks.
The website goes on maintenance mode during weekend. Try using during weekday. Also, always save application after filling up a page.

I have been having the same issue as well. No clue when or how to get it looked into.
I confirmed with VFS, there is NO requirement of SELF SWORN affidavit anymore.

QQ- how have people filled the affidavit-for-change-in-appearance-and-signature form? It just doesn’t allow to fill the online version, and if you save in PDF, when you enter information it overlaps with adjacent words…
Q1. Do we need to reproduce the form in WORD doc? And need to maintain the structure - photo box, signature box?
Q2. Also hoping we can paste/glue the photo in the box?
Q3. Guess for both below we need to select - “has changed” option ??
That my appearance has / has not changed.
That my signature has/ has not changed

Many thanks for all your guidance. My passport expires this year and I am applying to San Francisco consulate by Tatkal. My current passport was issued using my address in India where I lived earlier. However, currently, I and all my family live in the US. We still have the house in India at the same address, but the house is locked and no one lives there. My questions:

The online passport form asks for two addresses: Printable Address and Other address. I was thinking of having my US address printed on the passport and put India address in ‘Other’. Any issues or negative implications of this?

Does police verification require an actual house visit in India and a verbal confirmation from a resident of the house? Since my house in India is locked and no one currently lives there, I was wondering what would happen in my case.

They have mentioned that I can provide a relative’s address in India. My relatives in India live in other states (not in the state where my current passport’s India address is). Is it OK to provide that? The police station over there might not have any information on me.
Dates are getting closer, so I would really appreciate an answer from you or anyone else on this board who can help.
Hi @anil_am22, quick question – We are applying for renewal of my wife’s passport with US address proof. She has DL from a different state from where we reside(because she moved recently). So, can’t use it. And all the house and utility bills are on my name.
Can I use my address proof with our marriage certificate? will that work as address proof for her application?
- No. I can’t think of any. You would need at least 1 Indian address.
- Users have shared varied experiences on this. PV may or may not happen. In many cases Police visited the premises, while in others they called people to Police Station.
- You can give contact details of your friend/neighbor too and give them your documents, if PV happens.
did you get passport done ? I am trying to apply in tatkal while my USCIS application is pending. any idea if that is acceptable?

I have few question if someone can help? I am renewing my son’s Indian passport (Minor)
- As per the checklist It says that we need to share the Passport and Approval Notice but does not say self attested and notarized. However later in the checklist it says that passport needs to be self attested and approval needs to be notarized.
Q - Do we need to send 2 copies or 1 is fine?
- Does Checklist needs to be filled by us?
- My Lease is month on month so i am putting utility bill for address proof with Lease main page. Is that enough? or do we need to mention on sticky notes explaining Month on Month Lease.?
- In case of applicant residing with parents/children/ relatives, a notarized undertaking from them with one of the copies of address proof as indicated above. - What is this Document?
- We are not changing Indian Address - Do we need to share Indian Address Proof?
- Do we need to Self Attest the documents as checklist only says for Passport

I also made a mistake in the parents name section, should I create a new application with new email id?
If you have submitted application but you are yet to make payment/send it to VFS, create another one with correct info (with same email id)…
If you have already sent application, reach out to VFS…

Hello guys,
Do we know if a pre-authorization from Indian Embassy is still required in July-2021 for TATKAL renewal/reissue of Indian Passport and if the answer is a yes, how do we get that pre-authorization? Nobody at the Indian Embassy at Washington DC ever picks up the phone when you press ‘3’ on IVR for “Passport and Miscellaneous Services”. Can they be reached via email and ever respond to enquiries via email?

Hi @anil_am22 @Diwedy - I have a few questions regarding Child passport renewal:

In case of self attestation -
a. Does child need to sign parents documents as well like Father/Mother’s passport copies, I797 approval etc. OR only respective parent needs to sign their own documents?
b. Can only one parent sign all other child’s documents along with the various forms OR both of the parents need to sign? -
In case of notary -
a. Does parent need to sign as well along with the child OR only child’s sign is enough along with the notary officer? -
I can use my alternate email address to register my child with Indian Govt site. Correct?
Please do let me know on the above. Your help and support would be greatly appreciated.
