Hi Suja… I am also in the same boat like you… My 6 year old son who is in kindergarten also need to change Surname BLANK problem in the indian passport so that he can split the given name into surname and first name. Could you please say what documents from school will help him for this? So, in the case of very minors who dont have document, what they do ? Thanks in advance.

I could submit his high school transcript and his college document in his preferred name.In our case there was no change in his full name and we spilt his surname( 2 names) in to middle and last name. Thank you!!!

I also had so many questions but when I called, they cut the call in the middle and I was charged 24.8$.

My application rejected as I applied splitting name in TATKAL .
Splitting of name consider as DATA change and hence it cant be done in TATKAL .
I need to apply again in :frowning: normal processing .

MAKE SURE YOU APPLY IN NORMAL PROCESSING NOT IN TATKAL for split of NAME .

If you are in USA for more then 3 years then you are good with SSN ,USA DL or any other govt ID showing clearly first name “ABC” and last name " XYZ" .Most of India document is like ABC XYZ and they might not consider it alone but with SSN,USA DL etc you should be good .

Not yet …but i think timeline is 3 weeks on an avg .So will wait for atleast 2-3 weeks

Able to split name successfully . Here are few information .

  1. Spilt name should be in NORMAL processing
  2. Police verification done at India address provided .My relative done verification in INDIA.
  3. It took 11-12 working days to get NEW passport in Hand.
  4. Refer my earlier post for document supporting first name & last name .
  5. DONT staple documents not even your application form .Its written in instruction as well.

Hi all:

I have a question regarding “Affidavit For the Split Name Change”. Is “Affidavit For the Split Name Change” and “AFFIDAVIT OF NAME CHANGE” are same?

I have applied for the Passport Renewal. I have not changed my name. I have the EXACT same name on my passport renewal application as on my expired passport. I do not want to change my name. Once again I have the exact same name on my passport renewal application and passport.

Checklist is saying:
Remove/Change the Spouse Name:
Applicant has to provide supporting documents if there is a change or deletion of spouse name for
instance: Divorce decree or and new marriage certificate or and death certificate

I have my father’s name on my passport. I am in divorce right now. ( I am in the process right now so I don’t have a final decree yet). So, I don’t want to change, add or delete my spouse’s name. Besides, my spouse’s name was never on my passport. I have requested for the fresh passport with the exact same information. (My name and address are exactly the same both on my passport renewal application and expired passport).

I have attached my Driving License and GC with my renewal application.

I am not able to understand why I need to provide a Split Name affidavit in the application.

**I don’t know if any VFS officials read these messages. **
**If someone does, I want to request them to change the language on the checklist. **
Because checklist is not saying anywhere “Split Name” in fact it’s saying

"Supporting Documents- CONDITION BASED
Remove/Change the Spouse Name:
Applicant has to provide supporting documents if there is a change or deletion of spouse name for
instance: Divorce decree or and new marriage certificate or and death certificate.

I didn’t send this affidavit with my application because I didn’t change my name at all. And now they have sent me “Incomplete Application” email.

AFFIDAVIT FOR CHANGE IN NAME saying:

………………. (old name), doing……………………(give profession or vocation) and resident of
…(address) solemnly declare:

  1. That for and on behalf of myself and my wife, children and remitter issue , I wholly renounce/relinqui

I don’t have a new name. So, can I write my existing/old name at both the places?

Can you edit this form to make it user friendly?

Any reply/ insight would be sincerely appreciated.

Yes, I filled with pen over the space provided. I extended my name b/w lines and top of other words

*We would like to inform you that need to send two public documents ( provided aadhar, pan card) issued from the government or educational documents ( provided 10th and Bachelor’s degree ) in the desired name format it can be from the US or India also needs to have the same name format mentioned on the government application form. *

Please be informed that US public documents will only be accepted if the applicant residing in the USA continuously for over three years.

Please note need to send the Affidavit of the name change - Notarized.

Please be also informed that Bank statements and Credit card bill will not be accepted as name change documents.

I followed this trick to use same name everywhere on affidavit since it’s name split, it worked!

I had my name in desired format in 10th, college degree, provided that as educational documents with affidavit. Regarding initials I think it’s not allowed, Email vfs global they will assist you.
