Well ,Best option or my advice is … Dont do any thing ,simply wait for next visa stamping .
IN DS 160 ,you need to fill name as per passport( new passport) and then just below that option you have “use any other name”, so simply put old passport name it .Once you enter USA ,they will give I94 as per you latest DS 160 . I am assuming ,your SSN having correct split name .
Here is flow , passport name split ----> DS 160 as per latest passport —> visa should be issue as per DS 160 form ----> at POE, I 94 will be issue as per system which is name on visa .
Next I797, you can again put as per passport next time . Then everything will be aligned .
NOW if you are not planning for visa stamping in home country then check process with authorize attorney. Dont want to give advice which I have not face but I know its quite possible .
@Parryteacher do you have any suggestions what documents I can submit for elementary kid for split name?
DO you have EAD /AP for your kid ? If yes you can use it .
Otherwise ,I will suggest to check once with vfs ,they will guide you .Generally with under taking + one document like BC for kid with name separate ,should work.
Share your experience once you have success with kid’s split name .

Thanks for the reply!No we don’t have any . He is on H4. Only has ITIn that has FNU. VFS said 2 docs. BC has full name. VFS won’t accept even school attendance report (elementary)where his name is in desired format

Full name in the sense ? is it ABCXYZ or “ABC XYZ” in birth certificate ? how about name appear in visa or any immigration document like 797 ?

this is my personal suggestion : option 1: submit document with BC + one notarize under taking + one declaration on stamp paper stating first name and last name ( from india can be done by nearest direct relative like your father or mother ) .Give it a TRY .
option 2) wait for your DOF if you have time say 1-5 yrs .while submitting EAD + AP document ,you put first name last name as you want to display in your card then get SSN for kid based on that card .you will have two document . Best OPTION .
option 3) go through name change but I will not suggest this option personally .
hope this help .Do share your experience if you successfully able to split name with any of the option .
Sure, I have already submitted the documents (BC) but dint add declaration(as I sent before I got your reply ) I will see if that doesn’t work will try for second option.
I contacted Consulate they said to follow VFS directions

Split name for my son(9 year old) done successfully. Got the passport in a week. Applied in normal processing.
His Name was in
BC : ABC XYZ
Passport
Given Name: ABC XYZ
Surname: blank
All immigration documents ABC XYZ FNU
Chose the option change in particulars and checked given name and surname
Filled the form with desired format that is
Given name: ABC
Surname: XYZ
Mandatory docs and
Split name docs
He dint have any govt documents with desired format .
1.Affidavit notarized with 2 witnesses signature
2.Principal gave letter saying he is a student from start date to till date with screenshot of database record where his name was in XYZ,ABC
3.State department immunization name in desired format.
4. Letter to consulate mentioning our concerns and upcoming travel and visa stamping self attested.
Got his passport in 6 days (including Saturday and Sunday) that was so fast.

I am in the same situation. My surname is blank and given name has “firstname lastname”. I want the lastname to be added in surname in the passport. Ihave filled application with change of particulars in given name and surname. Please help me in what format the affidavit has to be filled and what documents are required. As seen in above posts, many of them were requested additional documents with first name and last name split and no Indian Govt doc has that format. What did you do in that case. Please help. US DL or SSN or EAD is having desire name format ? then you can use it .
Affidavit can be filled < GivenName Surname > in all places. I did this and had no problem. Supporting docs, like Indian pan card/ adhaar card (public), Bachelors/10th (educational) should be in GivenName Surname format

My wife and I are trying to figure out the process to get her name split in the passport. We tried splitting her name once via VFS last year, however, it got stuck at the requirement of 2 public documents with the desired name in the First and Last name format and so had to withdraw the application for a refund.
We are now trying to redo the process before traveling to India for Visa stamping and before the perm process starts and hence need your feedback or personal opinion on what is the best way forward as per your experiences.
Her name in all her documents is as shown below:
Passport- Given name: ABC XYZ
Surname: (blank)
All Indian Docs(except BC)- Name: ABC XYZ
Birth Certificate(In the process of getting this corrected from the Panchayat using “one and the same certificate”)- Name: ABC DEF
Visa- Given name: FNU
Surname: ABC XYZ
SSN(as per visa)- Full Name: FNU ABC XYZ
Expired US DL(issued as per name on SSN)- Given names: FNU
Family name: ABC XYZ
Renewed US DL(they no longer put FNU for some reason)- Given names: (blank)
Family name: ABC XYZ
From what I have heard, the process was pretty straightforward a couple of years back however, recently there is a requirement for producing 2 documents in the above-mentioned format and that is where we are facing this challenge.
Kindly let us know what would you suggest in this scenario.

If you have all your INDIAN docs in FIRST_NAME LAST_NAME format, then it should work fine for a passport renewal using this method:

All the Indian docs have the name mentioned in one line i.e. ABC XYZ and not as
Given Name: ABC
Surname: XYZ
VFS for some reason requires this format for the supporting documents.
This was why we had to take back our application the last time we tried splitting the name.
